Birth Certificates - Department of Health
The Report of Live Birth is often referred to as the original birth certificate. This is the original report filed with the Department of Health regarding an individual's birth. In accordance with the Vital Statistics Law of 1953, this document is not classified as a public document until 105 years after the individual's birth.

Birth Certificates - NYC Health - New York City
The NYC Health Department has birth records for anyone born in the city. Starting January 15, 2020, you can request a copy of a pre-adoption long-form birth certificate for an adopted person who was born in NYC. The long-form birth certificate is the detailed certificate on file with the agency that processes the birth record.

Birth Records | Utah Office of Vital Records and Statistics
